Health Insurance - Harvard Pilgrim Healthcare HMO 

Participation requires benefit eligible status and a minimum of 30 hours per week.  Individual, Individual +1, and Family plans are available.


Medical Expense Reimbursement Plan – Express Benefits

Employees who participate in the health plan are eligible for 75% reimbursement for inpatient and outpatient co-pays one time per year per covered individual.


Dental Insurance – Principal Financial Group

Participation requires benefit eligible status and a minimum of 20 hours per week.  Individual, Individual +1, and Family plans are available.  Choice of $1000 or $1500 annual calendar max.


125 Flex Spending Plan – Crosby Benefit Services

Medical Reimbursement:  $2500 max per year             Dependent Day Care:  $5000 max per year


Life, Short-Term and Long-Term Disability Insurances – Lincoln Financial

Provided free to employees who meet eligibility criteria. 


Life Insurance: 1x annual salary

Short-Term Disability: 7 day waiting period then 66-2/3% average weekly gross salary

Long-Term Disability:  After short-term is exhausted, pays 60% average monthly to a $4,000 max.


Earned Time Program

Combines vacation, holiday, personal and sick time into one bank.  Accrual starts at eligibility and rates are based on hours worked and years of service. ET is available for use after 90 day introductory period.


401(k) Retirement/Pension Plan – John Hancock

All employees age 21+ are eligible.  Up to 6% match annually.  100% vested after 3 years of service.
