What services are provided?

The Norwell VNA has a variety of highly skilled health care providers on staff, including skilled nurses, home health aides, certified diabetes educators, licensed social workers and many other knowledgeable health care professionals.

When is VNA care necessary?

VNA care may be necessary for a variety of reasons. We treat patients who are recovering from illness, surgery, stroke, heart attack or an accident. Our job is to service patients as well as their families, with an overall goal of maintaining independence and dignity. We accomplish this goal by empowering individuals with the tools and information they need to stay healthy and happy within the comforts of their own home.

How can I contact my nurse?
Our trained, caring clinicians have active schedules that often take them away from the office. However, we are always available to meet your needs. Call our office at 781-659-2342 in order to be connected to your nurse as quickly as possible.

For scheduling and billing questions, please call during office hours, Monday-Friday 8:00 a.m.-5:00 p.m.

What if I need help at night or on weekends?
If you need help during non-office hours, the Norwell Visiting Nurse Association has a 24-hour answering service, 7-days-a-week. We can always be reached at 781-659-2342.

Will my insurance pay for these services?
We will review your insurance benefits on your first visit. Norwell VNA accepts Medicare, Medicaid, HMO's, VA, private health insurance and private pay managed care.

When will the VNA call me?

Within 24 hours of being discharged from a hospital, rehabilitation center or nursing home, a nurse or physical therapist will contact you.

What happens on the first visit?

During the first visit, a nurse will assess your condition and develop a care plan. This plan may include nursing, rehabilitation, physical therapy, occupational therapy, nutrition, creating a medication schedule and/or counseling.

How can I take charge of my own health in between nurse visits?
We encourage our patients to keep track of vital signs such as blood pressure, weight, blood sugar levels and oxygen levels. Additionally, it is important to maintain a regular medication schedule, as prescribed by your physician. Following discharge, we may recommend disease-specific education and support group meetings, where you can ask questions and learn from others who are experiencing the same health issues. If anything changes in between visits, it is important to let your Norwell VNA clinician know!

What are my rights as a patient?

At Norwell VNA, your privacy is a priority. We follow strict federal and state guidelines to maintain the confidentiality of your medical information.

Additionally, you have the right to:
  • Request that we restrict how we use or disclose your medical information (we may not be able to comply with all requests)
  • Request that we use a specific telephone number or address to communicate with you
  • Inspect and copy your medical information (copying fees may apply)
  • Request additions or corrections to your medical information
  • Receive a report of how your information was disclosed (excludes disclosures for treatment, payment, healthcare operations and some required disclosures)
