The Norwell VNA has a variety of highly skilled health care providers on staff, including skilled nurses, home health aides, certified diabetes educators, licensed social workers and many other knowledgeable health care professionals. [Back to Top] When is VNA care necessary? VNA care may be necessary for a variety of reasons. We treat patients who are recovering from illness, surgery, stroke, heart attack or an accident. Our job is to service patients as well as their families, with an overall goal of maintaining independence and dignity. Our trained, caring clinicians have active schedules that often take them away from the office. However, we are always available to meet your needs. Norwell VNA accepts Medicare, Medicaid, HMO's, VA, private health insurance and private pay managed care. [Back to Top] When will the VNA call me? Within 24 hours of being discharged from a hospital, rehabilitation center or nursing home, a nurse or physical therapist will contact you. [Back to Top] What happens on the first visit? During the first visit, a nurse will assess your condition and develop a care plan. This plan may include nursing, rehabilitation, physical therapy, occupational therapy, nutrition, creating a medication schedule and/or counseling. [Back to Top] How can I take charge of my own health in between nurse visits? We encourage our patients to keep track of vital signs such as blood pressure, weight, blood sugar levels and oxygen levels. Additionally, it is important to maintain a regular medication schedule, as prescribed by your physician. Following discharge, we may recommend disease-specific education and support group meetings, where you can ask questions and learn from others who are experiencing the same health issues. We follow strict federal and state guidelines to maintain the confidentiality of your medical information.
