Overview of GCM

Our Geriatric Case Management Services start with a Geriatric Nurse Case Manager who works with the client and family, if appropriate, to assess and evaluate the client’s needs. Our Geriatric Nurse Case Manager offers clients personalized services including:

• Medical assessment and education
• Medication management
• Medical appointment coordination
• Coordination of professional services including nursing, physical and occupational therapy, nutrition, and social work
• Referral to para-professional services including home health aide, personal care aide, and companion care.
• Links to community and professional resources
• Liaison for family members and/or others involved in clients’ care.

Medicare does not cover Geriatric Case Management Services. However, many long-term care insurance policies will pay for such services. We will assist in reimbursement from long-term care insurance, if applicable.
