Telemonitoring
We are happy to offer telehealth monitoring with the Cardiocom monitor. This system allows patients to check their vital signs such as blood pressure, heart rate, body weight, oxygen saturation and blood glucose daily. The system can be customized to the patient’s specific needs and programmed to ask several disease related questions.
The data collected each day is transmitted via the telephone line to the Norwell VNA and Hospice telemonitoring specialty nurse for assessment. This allows for early intervention when a health problem is detected, providing patients and their families with increased peace of mind. Such monitoring also decreases both emergency room visits and hospitalizations.
