Norwell VNA and Hospice utilizes a variety of technological advances to enhance at-home care while encouraging patients to take an active role in their health. All of our clinicians carry pocket-sized, hand-held computers (known as the "PalmWyse" system). These instruments are carried into the field each day, and are programmed to include Medicare and other insurer forms, as well as confidential patient records. This virtually eliminates paperwork, allowing nurses and other clinicians to spend less time on administrative tasks and more time providing quality patient care.
In 2004, Norwell VNA and Hospice launched the HomMed Health Monitoring System, a groundbreaking clinical development allowing clinicians to track patients' health and well being on a daily basis from our Norwell office. This information can be shared with the patient's physician. A report is also generated for the patient, creating a seamless stream of communication. As a result, patients and their families enjoy greater knowledge and control over their health than ever before, while the incidence of emergent hospital visits is substantially decreased.
