What is Hospice?

At NVNA and Hospice, our hospice care focuses on the individual needs of our patients to improve their quality of life. Our team works with each patient’s physician to find the best approach for pain and symptom management. We provide every hospice patient with dignity and respect, and are always mindful of their needs.

How We Care

​

Hospice Team - Our patients always have a team behind them to ensure comfort and support. In addition to a nurse case manager, medical social worker, and home health aides, families can also request physical and occupational therapists, speech therapists, spiritual care coordinators, and volunteers.

 

Medical Care - Both our hospice medical director and hospice nurses are certified in hospice care and have extensive training in pain and symptom management. They will work together to provide an individualized comfort care plan based off the patient’s needs. 

 

Personal Care - Specially trained in hospice care, our home health aides provide assistance with personal needs, including bathing, dressing, skin care, and restroom assistance. 

Emotional - Our social workers provide counseling, grief support, and community resources for our patients and their families. 

 

Spiritual - Our spiritual care team at NVNA and Hospice cares for patients dealing with questions and concerns about the end of life. 

 

Bereavement - We provide support to family members for up to 13 months following the loss of a loved one.
