Presenters:
Nicole DePace MS, APRN, GNP-BC, ACHPN, Palliative Care Nurse Practitioner. Nicole’s role at NVNA and Hospice is the Director of Palliative Care consultations along with implementing best practices in staffing and care delivery according to available evidence. In addition, she maintains an active clinical practice providing palliative care consultations across care settings in the community (home, ALF, STR/LTC, clinic).
Karen Gore, LICSW, End of Life Certificate. Karen’s role is the Bereavement Coordinator for NVNA and Hospice and provides bereavement counseling, clinical supervision of social work staff, psychosocial assessments and education of hospice team. She also facilitates bereavement groups and workshops in the community.
Sarah Clancey, MTS, BCC. Sarah’s is the hospice house chaplain for the NVNA and Hospice and their Pat Roche Hospice House. She provides spiritual support for patients, families and provides training for staff on spiritual care for hospice and palliative patients. Her skills include hospital, hospice and palliative ministry.
Program is free for the nurses and other health care professionals of NVNA and Hospice. CEs would be for nurses only.
This is a two-day intensive multi-modal learning experience focused on providing hospice and palliative care clinicians with core competencies in caring for the patient and family living with serious illness. Completion of this course will enhance your ability to provide competent and compassionate care to patients and families through end-of-life and expertly address their physical, psychological, emotional and spiritual needs.
Goals:
The participant will demonstrate the knowledge and competence as defined by the Hospice and Palliative Nurses Association to deliver quality palliative and hospice care.
The participant will utilize core skills to address the myriad of needs facing patients and families living with serious illness and at end of life.
The participant will use effective communication strategies to describe palliative care and hospice to patients, families and colleagues.
The participant will explain clinical indicators for palliative care and hospice transitions.
The participant will identify factors that contribute to successful interdisciplinary team communication and collaboration.
The participant will explain all aspects of comprehensive symptom assessment.
The participant will select appropriate interventions to manage symptoms expected in serious illness.
The participant will demonstrate therapeutic communication skills.
The participant will design individualized care plans to treat distressing and non-distressing symptoms at end of life.

Post-Course instructor evaluations will be completed at conclusion of two day intensive course.
There will be no testing required of participants. There will be a pre and post-course knowledge evaluation.
Participants are expected to seek CHPN certification within 6-12 months of completion of the course.

As an American Psychological Association (APA) approved provider, CES programs are accepted by the American Nurses Credentialing Center (ANCC). These courses can be utilized by nurses to renew their certification and will be accepted by the ANCC. Every state Board of Nursing accepts ANCC approved programs except California and Iowa, however CES is also an approved Continuing Education provider by the California Board of Registered Nursing (Provider # CEP15567) which is also accepted by the Iowa Board of Nursing.
